Where do I find my pathology referral?

You can find your referral forms and barcodes in the Everlab App. Tap on your pathology appointment task to view and access your files.

Important: Bring Your Forms

For all pathology appointments, please ensure you provide the clinic with all pages in your form.

Please print all forms and bring them to your provider. If you cannot print them, ensure you have the digital copies ready on your mobile device to show the barcode to the pathology staff.

Is your referral “Generating”?

If you attempt to view your referral and the status shows as “Generating,” it means the system or your doctor is still finalising your referral documents and they are yet to be published.

If you need your referral immediately, please let us know at [e-mail]. We can manually prioritise the publication of your forms and ensure they are available in your Everlab App right away.
